Thank you for reaching out to Helping Hands for Heroes regarding transportation assistance. We are honored to serve those who have served us! Our service range includes Clay, Baker, Duval, St. Johns, and Nassau Counties
Please complete the request form below at least 48 hours before your appointment. Transportation is not guaranteed and based upon availability so as early as possible to secure a ride is preferred.
Levels of service options:
• Taxi = no assistance needed in or out of vehicle.
• Ambulatory = some assistance needed in and out of vehicle.
• Wheelchair = wheelchair assistance and lift needed.
Once we get your request, a dispatcher will contact you to complete your eligibility verification and to get a transportation waiver.
As part of the process, we will kindly ask that you provide proof of veteran or active military status.
Acceptable forms of verification include any one of the following:
• DD214
• Driver's License with Veteran Status
• Veteran ID Card (VIC)
• VA Health Identification Card
• Military ID
• Official VA Documentation
